1. Minimum Cum GPA requirement = 2.85
2. Spring Semester Sophomores & Juniors must have met 10th grade benchmarks

CONTENTS OF A COMPLETE UB APPLICATION

1. Completed Application, signed by parents
2. Copy of pages 1-2 of parent's annual tax form for the current year (1040, 1040A)
3. Copy of permanent resident card if student is an eligible non-citizen
4. Recommendation form completed by high school counselor including 8th or 10th grade benchmark scores
5. Copy of most recent high school transcript
6. Writing sample
7. Activity & medical release form
